Een uitstulping van de buikwand

Een 72-jarige man is onlangs opgenomen met een pancreatitis en herpes zoster binnen het 11de en 12de thoracale dermatoom. Enkele weken na ontslag heeft hij een pijnloze zwelling in de linkerflank. Wat ziet u?

Ik denk dat dr. Soeters nog even de verschillen tussen een myotoom en een dermatoom moet doornemen. Dat is niet hetzelfde. We zullen het zien.

The picture shows a swelling very proximal (far away from) above the 11/12th dermatome and signs of an old herpes zoster at the site of the 11/12th dermatome, so much more distal then the swelling. In view of the pigmentation and abscenc of red, fres...h vesiculae the Herpes Zoster must have started months or years earlier than the pancreatitis. The pancreatitis shows a rather typical chain of events, recovering slightly but then in the course of weeks of mild to moderately severe illness developing a pseudocyst. There is no true wall, but rather inflammatory reaction around the fluid collection containing amylase, debris etc, which when pressing on the muscle wall by way of its inflammatory effects paralyses muscle and leads to atrophy, allowing the fluid collection to become visible by lack of resistance by this unhealthy muscle.
So I bet there has been a Herpes Zoster, starting months or longer ago and now a pancreatitis rather recently (weeks) with the described symptoms. I have seen two of these cases but did not know or do not remember whether the two conditions could occur metachronically. Were there still manifestations of Herpes Zoster? I disagree therefore with doctor Bonte, who claims that the swelling is exclusively due to the Herpes Zoster.

@Bruin: Goede studies zijn er niet. Ik heb er nu in enkele jaren twee gezien, waarvan er een herstelde en de ander niet. N=2 dus. Vaak zijn het oude mensen, omdat die blijkbaar gevoeliger zijn voor een herpes zoster, zoals ook uit de literatuur blijk...t.

Prachtige foto! Dit is een radiculopathie ten gevolge van de herpes zoster met motorische uitval van de buikwandmusculatuur.
Dit heeft mij al eens een maand hoofdbrekens gekost bij een patiënt met dezelfde kwaal die helemaal binnenste buiten was ...gekeerd door chirurg en internist, maar waarbij de CT van het abdomen in retrospect prachtig de atrofie en parese van de buikwandmusculatuur liet zien. Kennen is herkennen! Ik vergeet het nooit meer.
